My mic is currently plugged into "Mic 1", my headphones are plugged into "Phones", and the mixer itself is plugged into the computer via USB.

It's not just voice communication programs either, recording in audacity or the built in windows voice recorder while listening to music or watching a video had the same result, my system sounds being recorded in addition to my voice. If I watch a video or listen to music while in discord with them they can hear it. POWER IN Use the included power adapter (18V DC, 800 mA, center-positive) to connect the MultiMix 4 USB FX to a power outlet. I was excited to show it off to some friends when I hopped into voice with them (discord) only to find that despite them being impressed with the audio quality, they could hear themselves and all of my system sounds. You could instead select the other "USB Audio Codec" for Playback Engine, but then you would only get input - no output (fine if you just need to capture something without monitoring then switch back to the other to play back).I'm using this setup primarily for doing some voiceovers as well as streaming and so far it's been great (note: i've only been using it for a day). (Still might need to Default your I/O in the Input and Output tabs.) So try launching Pro Tools with N and select Pro Tools Aggregate I/O for your Playback Engine. Be sure your audio software program is set up to receive audio via the MultiMix 4 USB. Pro Tools Aggregate I/O gets around this limitation by allowing you to combine the Input and Output of your CoreAudio device to treat it as one. If you are selecting "USB Audio Codec" then that is either just the Input or just Output of your Alesis (sounds like the latter based on your problem).

Be sure your audio software program is set up to receive audio via the MultiMix 4 USB FX. I have attached another screen shot to show what I see. I do have USB CODEC selected as the Playback engine. I initially DID have to start with the N key down because I was getting the prompt that the I/O was not recognized (or something like that).
